Severe protein C deficiency (i.e. protein C activity <1 IU dL−1) is a rare autosomal recessive disorder that usually presents in the neonatal period with purpura fulminans (PF) and severe disseminated intravascular coagulation (DIC), often with concomitant venous thromboembolism (VTE). Recurrent thrombotic episodes (PF, DIC, or VTE) are common.

Protein S deficiency: a clinical perspective

Protein S (PS) is an extensively studied protein with an important function in the down-regulation of thrombin generation. Because of the presence of a pseudogene and two different forms of PS in plasma, a bound and a free form, it is one of the most difficult thrombophilias to study. A deficiency of PS predisposes subjects to (recurrent) venous thromboembolism (VTE) and foetal loss. However, the conundrum of diagnosing PS deficiency has led to conflicting reports of PS as a risk factor for VTE. In this review, we aim to present a clinical perspective of PS deficiency.

Thrombosis is the medical term for an abnormal blood clot in an artery or vein. The body’s ability to form blood clots its natural defense against bleeding. Clots are formed through a series of chemical reactions between special blood cells (platelets) and proteins (clotting factors) in blood. This article discusses the treatment of Thrombosis.

THE GENETICS OF THROMBOPHILIA

Thrombophilia is a medical term used to describe the condition where the blood has an increased tendency to clot. There are many reasons why the blood can have this increased tendency. Thrombophilia is usually categorized into two types--acquired and inherited. In acquired thrombophilia, the abnormal clotting is usually related to a specific cause, such as prolonged periods of bed rest after surgery, trauma to the leg, or having cancer. People with inherited thrombophilia tend to form clots due to a genetic predisposition inherited from their parents. People with inherited thrombophilia may have a family history of relatives with abnormal or excessive blood clotting. This brochure will explain how genes play a role in blood clotting and are related to inherited thrombophilia.

MASAB RECOMMENDS EXPANDING NBCA PATIENT ADVOCACY TO INCLUDE ATRIAL FIBRILLATION

NBCA’s prestigious Medical and Scientific Advisory Board (MASAB) met in San Francisco on December 7 to discuss projects for 2009, led by Jack Ansell MD, MASAB Chair. MASAB voted to recommend expansion of NBCA’s outreach to patients with atrial fibrillation (a fib). Up to now, a fib patients were not considered part of NBCA’s constituency because a fib-related clotting occurs in the arteries, not the veins. However, there is an overriding commonality with venous clots and our current patient constituency, as all face the challenge of life with anticoagulants, an important consideration for the NBCA Board when it weighs the merits of MASAB’s recommendation.

The CMS Ruling on Venous Thromboembolism After Total Knee or Hip Arthroplasty

This is an in-depth analysis of the implications of the October 2008 “Never Event” decision by the Center for Medicare and Medicaid (CMS) to withhold reimbursement for any patient who develops a deep vein thrombosis (DVT) after hip or knee replacement. Since NBCA is a patient led organization, it immediately expressed reservations about the zeal in such a “black and white” policy, since it overlooks the “grays” in the fact that clots may ensue when there is appropriate thromboprophlaxis given, or even withheld appropriately due to contraindication in certain patient populations.

NBCA Statement on Recent Statin Drug News Concerning Blood Clot Treatment

The results of this study are highly interesting. Statins have a number of effects other than lowering lipids and this may explain the positive effect seen in this trial on the rate of venous thromboembolism. However, before deducting anything clinical from this trial it is important to keep in mind that this was a primary prevention trial in a healthy population, not a secondary prevention trial, i.e. the trial did not investigate whether statins decrease the risk of (a) venous thromboembolism (VTE) in people at risk for VTE or (b) people who had already had one VTE. Thus, no comments are possible on (a) whether statins are an alternative to traditional DVT prophylaxis (heparin, low molecular heparins, Fondaparinux, warfarin, and the new oral anticoagulants) for people undergoing surgery or being hospitalized, and (b) whether they have any positive effect in people with a previous VTE and, thus whether they might supplement or be alternatives to warfarin.

Inherited Antithrombin Deficiency

Antithrombin (AT) is a potent inactivator of thrombin and factor Xa and the major inhibitor of blood coagulation. Inherited AT deficiencies are uncommon, with prevalences in the general population between 1 in 500 and 1 in 5000. They are either quantitative (type I) or qualitative (type II). Type II is subdivided into the more common, but less thrombogenic, type IIb deficiency caused by a defect in the heparin-binding region of AT and the less common, but more thrombophilic, type IIa variant caused by mutations in the thrombin-binding site. A pleiotropic type IIc deficiency also exists. In the evaluation of a thrombophilic individual, a functional AT assay (AT activity) should be used and the diagnosis of AT deficiency only established after acquired causes have been ruled out and repeat AT testing on an additional sample has been performed. A subsequent antigenic AT assay result leads to differentiation between type I and type II deficiency. Further specialized tests help subclassify the type II deficiencies, but this is typically not carried out for clinical purposes, even though it might be helpful to assess thrombosis risk. AT deficiency is associated with an increased risk for venous thromboembolism (VTE) and pregnancy loss.

Thrombic Storm - a Rare, but Serious Unknown Blood Clot Disorder

While most people who develop a blood clot in their vein have a deep vein thrombosis (DVT) or a blood clot in their lung (pulmonary embolism), a very small number of people who develop blood clots have a more serious and often life-threatening syndrome called Thrombotic Storm. People with Thrombic Storm experience more than one blood clot in a short period. These clots occur in different and sometimes unusual locations in the body. This very aggressive and serious form of blood clotting is often difficult to treat, and there is very little research or documentation of this disorder.

Clinical Corner: Cancer and Blood Clots

Cancer is an independent risk factor for VTE. Patients with cancer are approximately four times more likely to develop VTE compared to the general population3,4. However, it is important to recognize that this risk varies depending on the type of cancer and certain cancers are associated with higher risk for VTE than others. Cancers associated with the highest risk for VTE include brain cancer, pancreatic cancer, stomach cancer, ovarian cancer and hematological (blood) cancers like lymphoma and myeloma [specifically myeloma treated with certain types of chemotherapy]5,6. Examples of cancers that are associated with relatively low risk for VTE include breast and skin cancers.
